Requesting Accommodations
At the university level, disclosure of a disability is voluntary. In general, documentation of disability should only be submitted to the coordinator of student accessibility services.
Process to Request Accommodations
Follow the below process to request academic accommodations.
- Complete and return the Disability Accommodation Request and Release of Confidential Information [pdf].
- By mail: Coordinator of Student Accessibility Services

- The student must provide appropriate supporting documentation for academic accommodations. Please refer to the Supporting Documentation Guidelines for Students with Disabilities section for specific detailed information. The University's Guidelines for Documentation of Disability are adopted from the Association on Higher Education and Disability (AHEAD).
- Once the items from 1 and 2 (above) are received, the student will need to meet with the coordinator of student accessibility services to review the requests, outcomes, and to learn about how to utilize approved accommodations at Fisher. To schedule a meeting, call (585) 385-5252 or email sas@sjf.edu.
Note:
- The cost of providing the required documentation is the responsibility of the student.
- The final determination for providing appropriate and reasonable accommodations rests with the University and is made on a case-by-case basis based on the supporting documentation.
